I don't have experience with chronic pain, and though I rarely ever get headaches, I have had migraines in my life. There is nothing scientific about my analysis of my own migraines, but here are my observations from when I got migraines:
For me, migraines always occurred in the winter, when the temperature was below freezing and the air was super dry. This made me dehydrated in general, but it especially dried out my nasal passages a lot (also causing nosebleeds, btw).
I think that, for me, having dry and irritated nasal passages is what triggered my own migraines (along with the use of pseudoephedrine "Sudafed" - excellent at battling sinus congesion, but it always goes too far!). Therefore, whenever the weather gets cold, I take care to drink a lot of extra water, I don't blow my nose very hard (avoiding nosebleeds), we run a humidifer in our bedroom at night, and I keep a bottle of sterile saline nasal spray handy to make sure my sinuses don't get too dry. I also scarf up when going out in bitter cold temperatures, because the temperature differential can shock the sinus passages, and I'd rather look like a metro-goober than have a migraine.
I don't know all the cause/effect relationships, of course, but I can say that I haven't had a migraine since I started all this particular regimen.
