I don't think "week to week" is a fair assessment, but, yea, they apparently admitted to not even deciding who the final 5th was until after the S3 finale was filmed. Apparently the Final 5 idea didn't come to them until S2. Leoben ran away, abandons Kara (whereas he's always fought to remain by her side and always been supportive) after seeing her ....clone? dead Cylon body? and learns what the Hybrid said to Kara.
Why?
He wasn't afraid of Kara after learning she was a Cylon he only got afraid after learning about the Hybrid prophecy.
Kara definitely needs explaining. As far as the ressurection hub, I imagine even if it hadn't been blown up yet (I think you're right, BTW) she still would've been out of range to ressurect anyways.
Of course I'd feel a lot better and maybe take more time to figure these things out if I didn't read once that the writers just make up the details from week to week and that there was no ending planned.

RDM, as he did on DS9 generally at least plots at least a season in advance (DS9, I believe he plotted S5-7 before S5 was filmed)
