- when she shifted the mirror to escape to the Closed Fist?
- when she shifted the mirror to her dungeon location?
- when she took out Eremis?
This brings us up to a minimum of 13.
And I'm sure I'm still missing something. Darn it, pretty much 3/4 of the story is her getting into trouble and being rescued by someone...

My thinking is whenever Terisa is doomed until someone comes to her rescue. Simple enough, right?

You mean when the roaches almost ate them? The attack was aimed against Geraden but if Artagel and more importantly Havelock didn't help they'd both be dead. Also, Geraden and Terisa were constantly together so it's likely she'd die even if she didn't go to the lower levels. So I counted this one.

No. This is general mayhem. Though who knows how good communications between Eremis group and the wolves+firecats are. I seem to remember they locked eyes with Terisa? Maybe Eremis gave them Terisa's dirty undies to sniff. So... possibly?The Wolf/Firecat attack at Houseldon?

Well, we've got the furry spider pacman things in the courtyard, but I didn't originally count that because she walked into it eyes wide open, with help just for the occasion.
Gereden "rescuing" her from her old life?
Havelock pulling both she and Gereden back from the dinner party?
The "rescue" I attributed to Eremis was (IIRC) in his chamber. He was seducing Terisa (or trying, but being interrupted) Gart popped in, but there were witnesses I think, so Eremis threw a pillow at him, which let him know that this wasn't a good time.

I think I'm pretty much tapped out. Over all, I don't think I did too bad for not having read the books in several years. We've already got a couple apparent forfeits. Any others I try get hazy around the edges - for example, did the servant who taught her checkers ever save her from the closet? Those kinda blur together for me, but I don't think so.

Oh, the Servant! Forgot about him. From what I recall he wasn't a house servant. Maybe a gardener or a chauffeur? He was her only childhood friend so that must count for something.
After you recounted so many I'll give you hints:
- Life&death rescue happened in 2nd book during her travels.
- metaphorical one (though in retrospect could be a life&death) happened in 1st book and was hilarious.
