Twisting the Flicks ( the super spoiler thread)
Give us the best plot twists ever, your number one what the f@(k? moment from the movies. For me it was Fight Club the moment we are clued in on the true nature of Tyler Durden. I should have seen it coming but I didnt, or how about Angel Heart when we find out that Deniro is really Satan?

I don't know about my all time biggest shock ever. Couldn't pick one. In recent years, the best twist I've seen was the one in Old Boy when O-Dae Su finds out who his daughter is.

Yah, Usual Suspects was a kick in the teeth... I thought for sure is was .. in fact i WANTED it to be!!! 
The Prestige had some pretty shocking twists...
OH! The ultimate .. especially when I saw the remake first: Ocean's 11 with Frank Sinatra and Co. Anyone see that...? Wow, that ending shocked me... I stared blankly at the credits for minutes after it was over and then let out a huge, joyous belly-laugh ... fantastic ending!!!
On a very similar note, the original version of the Italian Job shoked and pleased me too (again, especially after I saw the remake first...)

OH! The ultimate .. especially when I saw the remake first: Ocean's 11 with Frank Sinatra and Co. Anyone see that...? Wow, that ending shocked me... I stared blankly at the credits for minutes after it was over and then let out a huge, joyous belly-laugh ... fantastic ending!!!

A Scanner Darkley was, for me, moment after moment of delicious surprise. Pan's Labyrinth also stands out as one of those movies.
The End of 12 Monkeys, where we realize Cole has been seeing his own death in his dreams the entire movie...
I LOVE seeing movies where I don't know exactly what is going to happen after 20 minutes. Some great examples of movies in this category...

Eternal Sunshine of the Spotless Mind
(The aformentioned)

Princess Mononoke
Stardust
Vertigo
A Clockwork Orange
Minority Report
Munich (I knew nothing about the events depicted. I think it's also one of the best thrillers ever made)
No Country for Old Men
Signs (Underrated IMO)
I think this should tell you something about my taste in all art. I love being actually enthralled and swallowed into something, which all of these movies did. Not knowing what is going to happen next, and being genuinely engaged in everything that's happening... It's a rare and exhilarating feeling for me. It happened when I read the Gap (and to a lesser extant TC)

Y'know, I'm right there with you on so many of these. If I may, I think Spirited Away belongs on this list. I think that was the first movie where I realized having no idea where the plot is going was so much fun, but had never realized it before. I think the movie Fearless was also one of those movies for me.
As for twists, a classic one is definitely the original Wizard of Oz.
But for me, what comes to mind that hasn't been mentioned is the Crying Game. It almost took an explanation from my girlfriend at the time for me to get it. Must also say Jacob's Ladder when it came out.
